Mr. Konur's treatment plan and preoperative condition are as follows:

Mr. Konur presented with history of pain discomfort on the upper right side, upon examination following were the findings: deep decay with pulpal exposure on tooth #03 and recurrent decay in tooth #02.  Teeth #2-3 were treated to alleviate pain with appropriate treatment.  After endo treatment for tooth #3 and core build up for #2-3 comprehensive examination was conducted.

Principal concerns were attrition, erosion possibly acid erosion due to acid reflux. No definite history of GERD, patient was advised to consult with his physician to run investigative tests.  History of bruxism/clenching in the past.  These conditions had led to loss of vertical dimension.

After smile evaluation, centric relation and TMJ evaluation treatment plan was formulated.
  1. Correct vertical dimension and restore centric relation with the help of orthotic removable appliance that patient will wear for 6-8 weeks so the correct and comfortable V.D. and centric relation is established.
  2. Place lab processed temporary restoration/full mouth to newly established V.D. and correct C.O.. Checked and corrected discrepancy using a leaf guage.
  3. Once patient was happy and comfortable with the temporary restoration and bite was balanced to a new vertical dimension permanent restorations fabricated by the laboratory and cemented.
  4. Fabricate bite guard to protect any future damage to the restorations.
